Gait Retraining in Parkinson’s Disease: A Cognitive Cueing Approach
نویسندگان
چکیده
Parkinson's disease (PD) is characterized by resting tremor, bradykinesia, rigidity, and postural instability. These cardinal symptoms commonly affect gait performance and therefore researchers have been investigating techniques to manage and treat gait impairment in individuals with PD. This thesis explores a progressive approach to gait rehabilitation using a novel intervention that combines traditional gait cueing techniques with motor learning approaches to facilitate self-cued gait improvement. Five participants with PD-related gait impairment completed the home-based gait training intervention. This intervention included video footage of each individual participant walking with and without verbal instructional cues, audio coaching, and practice periods. Participants were given the video to practice at home for two-weeks. Kinematic parameters of gait were assessed pre-intervention, at two weeks post-intervention, and at two months post-intervention. Results indicate that individuals with PD are capable of learning verbal-cueing strategies and utilizing these to generate long-term gait improvements through self-cueing. Each of the three co-authors contributed to designing the research methodology used to test the gait retraining intervention highlighted in Chapter 2, and facilitated the process of data collection. Dr. Spaulding also made valuable contributions to the process of data analysis and both Dr. Spaulding and Dr. Jenkins offered valuable improvement to this thesis through the process of iterative editing. Participants in the study (Chapter 2) were recruited from the outpatient neurology practice of Dr. Jenkins, without whose support this project would not have been possible.

A novel cognitive cueing approach to gait retraining in Parkinson’s disease: A pilot study
Background: Parkinson’s disease (PD) impairs gait performance, which can lead to falls and decreased quality of life. This study examined the feasibility of implementing a novel home-based intervention designed to elicit gait improvement in individuals with PD. Methods: Five participants with PD completed a two-week home-based gait retraining intervention designed around guided video feedback. ...
متن کاملEffect of On-Demand Cueing on Freezing of Gait in Parkinson’s Patients
Gait disturbance, particularly freezing of gait (FOG), is a phenomenon that is common in Parkinson’s patients and significantly contributes to a loss of function and independence. Walking performance and number of freezing episodes have been known to respond favorably to sensory cues of different modalities. However, a topic that has so far barely been touched is how to resolve freezing episode...
متن کاملRhythmic Auditory Stimulation (RAS) and Motor Rehabilitation in Parkinson’s Disease: New Frontiers in Assessment and Intervention Protocols
Previous studies have demonstrated that physical therapy accompanied by Rhythmic Auditory Stimulation (RAS) can improve the motor skills of patients with Parkinson’s disease and, in particular, their gait disturbances. In the present work we describe the neurological bases and perceptual-motor deficits generally associated with Parkinson’s disease, with a specific focus on gait disturbances. Wi...
متن کاملCueing Effect of Gait Ability in Parkinson’s Disease
The idiopathic Parkinson’s disease (PD) results from a degeneration of dopamine-producing cells in the substantianigra which leads to cardinal symptoms of hypokinesia, bradykinesia, postural instability, rigidity and tremor. These symptoms lead to difficulties in motor performance such as gait ability and falls. Even optimal medication therapy, gait problems associated with PD are often charact...
متن کاملMusically Cued Gait-Training Improves Both Perceptual and Motor Timing in Parkinson’s Disease
It is well established that auditory cueing improves gait in patients with idiopathic Parkinson's disease (IPD). Disease-related reductions in speed and step length can be improved by providing rhythmical auditory cues via a metronome or music. However, effects on cognitive aspects of motor control have yet to be thoroughly investigated. If synchronization of movement to an auditory cue relies ...
